Making a Reservation

Please complete the Field Trip Worksheet to quickly and easily submit your reservation online.

You can call the reservation office at 773-684-1414 to speak with a customer service representative. Please be prepared with the following information:

• Number of children in your group
• Whether you are interested in general admission only, general admission with Omnimax® or 3-D movie, and any up-charge exhibits
• Preferred dates and times (Omnimax® seating is limited, so please call early)
• Lunch plans for the group

Reservation Confirmation

You will receive a confirmation packet within six to eight business days. The packet will contain a Visitation Agreement, Group Visit Confirmation, bus driver arrival and drop-off information and a list of what to expect when your group arrives at the Museum. If you have not received a confirmation packet within six to eight business days of submitting your reservation, please contact the Museum by calling 773-684-1414.

The Visitation Agreement must be signed by the group leader and returned to the Museum upon check in on the day of your visit. Please be sure to bring your Group Visit Confirmation the day of your visit to ensure smooth entry into the Museum.

Payment

Full payment is due 14 days in advance of your group visit unless you have made special arrangements with the Museum during the advance registration process. Please note: Without full payment 14 days in advance of your visit, your reservation will be cancelled.

Payment Options

You may call the Museum at 773-684-1414 to pay with a credit card. Accepted credit cards: American Express, Diners Club, Discover Card, MasterCard and Visa.

Money orders and checks from an organization, school or teacher/group leader should be made out to: Museum of Science and Industry. Please mail via overnight mail to ensure safe and timely delivery. Checks and money orders can be mailed to:

Final Group Count

Teachers/group leaders must call in or fax the final group count to the Museum 14 days prior to arrival. Any students and/or chaperones not included in the final count will be charged regular, full-price admission.

Cancellation

The Museum can accomodate a limited number of students per day. Should you need to cancel or reschedule, please notify the Museum as soon as possible. Cancellations made less than 14 days in advance will not be refunded.
